What's the difference between Ozempic and Mounjaro?

Introduction

In recent months, what the media has been calling the "Ozempic of the rich" has gained prominence due to the impressive weight loss results shown by various celebrities. However, a common question arises: what is the difference between Ozempic and Mounjaro? Which is more worthwhile?

What is Ozempic?

Ozempic is a drug that was initially developed for the treatment of type 2 diabetes, but has shown great results in weight loss. It is an agonist of the GLP-1 hormone receptor (glucagon-like peptide type 1). GLP-1 is an incretin hormone that helps regulate blood glucose levels by stimulating insulin secretion and inhibiting the release of glucagon, which lowers blood glucose. Studies show that Ozempic can lead to significant weight reduction, as well as controlling glucose levels.

GLP-1 also acts by delaying gastric emptying, which contributes to weight loss. By slowing down the emptying of the stomach, it prolongs the feeling of satiety after meals, reducing total calorie intake. This combined action of controlling appetite and regulating glucose levels makes GLP-1 agonists an effective option for both the treatment of type 2 diabetes and weight loss.

Although it has not yet been approved for the treatment of obesity in Brazil, it has been used off-label due to the great evidence for weight loss. Its "brother", Wegovy, which is not yet marketed in Brazil but contains the same active ingredient as Ozempic, semaglutide, is already approved for weight loss in some other countries.

What is Mounjaro?

Mounjaro, similar to Ozempic, is also used in the treatment of type 2 diabetes and weight loss. It is a dual agonist of GLP-1 and GIP (gastric inhibitory peptide) receptors. Like Ozempic, Mounjaro works by stimulating insulin secretion and inhibiting glucagon, as well as improving insulin sensitivity and reducing appetite.

GIP is an incretin hormone which, as well as stimulating the release of insulin, also has direct effects on reducing food intake and increasing fat burning. This combined action of GLP-1 and GIP is unique to Mounjaro and is not present in Ozempic or Wegovy. GIP acts synergistically with GLP-1 to boost weight loss and glycemic control, providing a more comprehensive and effective approach to the treatment of type 2 diabetes and obesity. The inclusion of GIP in Mounjaro offers an additional advantage over drugs that only mimic GLP-1.

What's the main difference between Ozempic and Mounjaro?

The main difference between Ozempic and Mounjaro lies in the hormones they mimic. Ozempic is an exclusive agonist of the GLP-1 receptor, while Mounjaro is a dual agonist of the GLP-1 and GIP receptors. This dual action of Mounjaro offers an additional advantage in weight reduction and glycemic control. Comparative studies indicate that Mounjaro can lead to greater weight loss compared to Ozempic due to its combined action.

In terms of weight loss, while Ozempic has shown losses of up to 15% of body weight, Mounjaro can lead to an average loss of up to 20%, with some more advanced studies showing results of up to 27% of loss. These Mounjaro results are so promising and impressive that they compare to those of bariatric surgery, which has an average overall weight loss of 25%, without taking into account the specific type of surgery performed.

Is Mounjaro right for me?

Mounjaro may be indicated for patients with type 2 diabetes who are unable to control their glucose levels with diet and exercise alone. It can also be recommended for people who are overweight or obese (BMI ≥ 27 kg/m² with comorbidities or BMI ≥ 30 kg/m² without comorbidities). It is essential that the indication and monitoring be carried out by a specialized doctor.

Because it's an expensive drug and still unavailable in Brazilian pharmacies, you need to consider a minimum of US$1,200 (dollars!) per month of treatment, for at least 6 months (depending on your weight, more time may be indicated). Ozempic costs around R$1,200 (reais) per month.

It is essential to check with your doctor which option is best for you and how much weight you need to lose, as it is a long-term treatment and requires lifestyle changes, such as diet and physical activity, so that you can take advantage of the drug's full potential.
